Please note: When billing ALOXI using J2469, the billing unit is 25 mcg.
ALOXI should be billed at 10 units for a 0.25 mg dose.

These codes are used to bill for drugs and associated procedures performed in an outpatient setting.

Product Coding

J2469 is the permanent product code for ALOXI®.


National Drug Code (NDC)

ALOXI (palonosetron HCl) injection 0.25 mg and NDC: 58063-797-25. If an 11-digit NDC is required, use 58063-0797-25. 

NOTE: Many payor systems use an 11-digit NDC number. Confirm which code your carrier prefers. 

Administration Coding (CPT-4 code)

Form CMS 1450 (UB-92): Use C8950 for IV infusion therapy or C8952 for an IV push other than chemotherapy (applicable to hospital outpatient setting).

Form CMS 1500: Use 90767 for IV infusion (replaces G0347) or 90775 for IV push (replaces G0353) administered prior to chemotherapy on the same day.

*ALOXI is used in adults to help prevent the nausea and vomiting that can happen on the day of treatment with certain chemotherapies or occur days later following certain chemotherapies moderately likely to cause nausea and vomiting.

Tell your healthcare professional if you are taking other medications or if you have or develop severe heartbeat changes. The most common side effects of ALOXI are headache and constipation.
